So I caved in and made an appointment at KL Fertility! I had emailed
Dr Natasha a few times prior and she was so helpful and personable via email, I thought that maybe a second opinion would be good. After all, we were already on our 2nd Clomid cycle.
KL Fertility is located on the 6th floor of Wisma Perintis, in Damansara. The location is a bit hard to get to as there is a constant traffic jam on the road directly in front of the building - Jalan Dungun. But otherwise, Waze got me there just fine.
We made an appointment for 11am and thanks to the traffic jam all over KL, we got there right on time. The registration was fairly simple, and then we were told to wait. I think I waited for like 10 or 15 minutes only, maybe even less than that, before I was called in. At this point, I was already loving this place - the receptionist/Staff Nurse was friendly, unlike the sour-faced lady at Dr Selva's who did shout at me when I asked her a question (before she found out I was a doctor).
The first consultation with Dr Natasha was more detailed than my previous consultations - she started off asking us about why we were there today. I rambled about my irregular cycles and being on Clomid. Then she did start to take a history about our medical history - my PCOS, cycles, whether I ovulated on the previous cycle (I don't know, since we didn't do follicular tracking). She also went through all our (my) results. My fasting insulin level was HIGH. I had LOW vitamin D. My husband's SA also was not that satisfactory, according to her, since the morphology was only 0.5% in November. We did mention to her that our previous doctor reassured us regarding the morphology, so we were not quite sure which to believe. She recommended that we repeat the SA on that same day.
After chatting for about 10 minutes, she performed a TVS for me. My Left ovary had an irregular shaped follicle, so it was suspected that I had just ovulated perhaps the day before (darn it!). There were a couple more follicles on my right side, about 16 and 14mm so she told us that we could try our best for the next few days.
We went for lunch break and returned afterwards to review the results of my husband's SA. Unfortunately, the morphology was still low - 1%. An improvement from before, but still far from the recommended >4%. She told us the bad news : That it was highly likely that we would need to undergo not just IVF but ICSI-IVF. Yikes!
Of course, an IUI is not completely out of the question, and some patients DO get pregnant even with abnormal SA it's just that the chances are lower (15-20%) as compared to an IVF. According to Dr Natasha, at my age (27) if we did proceed with ICSI-IVF, our chances could be as high as 60-70%. Anyway she also said that hope was not lost yet - we would have to return in 3 months for a repeat SA. She prescribed a multitude of vitamins and supplements to improve my insulin resistance and hubby's sperm quality.
For me :
1) Multivitamin with Folate 600mcg
2) Vitamin E
3) Vitamin C
4) Fish oil
5) Metformin XR 1.5g ON
6) Vitamin D 1 drop ON
7) Calcium 600mg BD
For hubby:
1)Multivitamin
2) Tribestan 1/1 OD
3) Coenzyme Q10 100mg OD
4) Vitamin E 400U OD
5) Vitamin C 500mg OD
Needless to say, all these supplements cost a lot of money. :(
Summary of what we paid yesterday (at the clinic)
Glucophage XR RM 60.00
Vitamin D RM140.00
Semen Analysis RM100.00
USG RM100.00
Total RM 400.00
We lucked out of a consultation fee, thanks Dr Tasha!
So today is Day 1 of taking those vitamins. I feel like one of my patients...
Baby dust to everyone!
